Clinical Analysis after Surgical Management of Intrahepatic Stone



Abstract
Intrahepatic calculi or hepatolithiasis was defined as stones involving the right and left hepatic ducts and their branches, peripheral to their junction at the hepatic hilum, although the junction may lie outside of the liver parenchyme.
Therefore, any
stones present peripheral to the origin of the common hepatic duct or to the confluence of the main hepatic ducts are called intrahepatic calculi.
The authors reviewed 88 cases of the intrahepatic sotne from Jan 1987 to May 1991 in Departemtn of surgery, Hanyang University Hospital and the results were as follows;
1) The peak incidences were occured in 5 th decade (22 cases, 25%) and 6 th decade (28 cases , 31.8%)
2) The male to female ratio was 1:2.
3) The most common clinical symptom and sign were RUQ pain 980 cases, 91%) and RUQ tenderness (78 cases, 89%).
4) Sotnes were found mainly in both hepatic duct (40 cases, 45.4%)
5) The ooperative procedures were choledocholithotomy (25 cases, 28.4%) choledochoenteric anastomosis without partial hepatectomy (29 cases 33%), left lateral segmentectomy (20 cases, 22.7%), partial hepatectomy with choledochoenteric
anastomosis

cases, 9.2%), left lobectomy (3 cases, 3.5%) and right lobectomy (1 case, 1.1%)
6) The most common operative findings were CBD and IHD dilatation
7) The pathologic findings were pericholangitis (31 cases, 96.8%), periductal fibrosis (22 cases. 68.7%), cholangiocarcinoma (2 cases, 6.2%) and primary hepatocellular carcinoma (1 case, 3.1%)
8) Postoperative complications in hepatic resection group were wound complication (13 cases, 14.7%), pulmonary complication (10 cases, 10.2%), residual stones (7 cases, 7.8%) and in nonhempatic resection group were residual stones (27 cases,
21.8%),
wound complication (8 cases, 9. 1), sepsis (5 cases, 5.6%), in order of frequency
9) The cause of death was sepsis (5 cases, 5.6%) and there were found in nonhepatic resection group
